Why and How Often Is Revision Surgery Necessary after First Metatarsophalangeal Joint Arthrodeses? A Cohort of 120 Consecutive Cases

Background: Little is known about the rate and reasons for revision after primary Metatarsophalangeal (MTP1) arthrodesis with latest implants. While it commonly assumed that nonunion accounts most reoperations, we hypothesized malposition leading cause of revision. The aim this study was to determine MTP1 using cup- cone-reamers locking plate technology. Method: Between 2015 2017, 120 consecutive fusions in 114 patients were performed a low profile, pre-contoured titanium dorsal plantar metatarsophalangeal screw. within minimum one-year follow-up period (average 16 months [12 - 26]) index procedures documented analyzed. defined as any reoperation following procedure excluding hardware removal. Results: Seventeen feet (14%) underwent Four developed (3.3%) needed revision, 11 (9%) required painful malposition; insufficient extension excessive valgus accounted majority. Two medial sesamoidectomy. No infection or wound healing problems occurred. Twelve requested Conclusions: Nonunion fusion technology low. followed by nonunion. Excessive account symptomatic malposition. A recommendation 0 10 degrees horizontal plane may contribute diminishing rates; no conclusions could be drawn regarding an optimal position sagittal plane.
